Nomination Form fill-up and Nominee

Nomination is a Form where an employee hires or Nominates a member of his family or a known person or mentions his name as a Mentioning a name or alternative so that the employee can easily get the Nominee due after the death of that employee. And nominee is a person who has been proposed or formally entered as a candidate for an office/ institution as a recipient of a grant or award. Nomination মানে হলো এমন একটি ফর্ম যেখানে কোন কর্মচারী তার Family এর কোনো সদস্য বা পরিচিত কোনো ব্যক্তিকে নিযুক্তকরণ বা মনোনয়ন বা Mentioning a name বা বিকল্প হিসেবে তার নাম উল্লেখ করা যাতে ঐ কর্মচারীর মৃত্যুর পর কর্মচারীর প্রাপ্য অর্থ কর্মচারীর Nominee সহজেই পেতে পারেন।Nominee is a person who is proposed or formally entered as a candidate for an office/institution as the recipient of a grant or award but who is not an actual owner. Bank Account holder এর Nominee এবং General Provident Fund (G.P.F) এর Nominee একই  প্রকার, একই কাজ।

Apart from wife, the Employee can Nominate one or more Members

নিজের স্ত্রী (Wife) ছাড়া যাদেরকে Nominee করা যাবে, তারা হলেন-
(i) Sons including stepsons and adopted Sons.
(ii) Unmarried/ widowed daughter including step and adopted Daughter
(iii) Mother
(iv) Father

Nomination for Primary Teacher

Primary Teacher দের এই Nomination Form দুই কপি পূরণ করে Concerned SI/S দিয়ে Signature করিয়ে একটি original এবং অন্যটি duplicate Service Book এর সাথে Staple করে রাখতে হবে। Primary Teacher দের ক্ষেত্রে original Service Book সংশ্লিষ্ট SI/S-এর নিকট এবং duplicate Service Book শিক্ষক/শিক্ষিকার কাছে থাকে।

Nomination for High School Teacher

High School এর শিক্ষক/শিক্ষিকাদের ক্ষেত্রে Headmaster (H.M)/ Teacher in Charge (T.I.C)/ Assistant Headmaster (A.H.M) Managing Committee (MC) Resolution/ Administrator Order করবেন এবং Resolution/Order copy সহ দুইকপি Nomination Form একইভাবে original এবং duplicate Service Book এর সঙ্গে Staple করতে হবে।High school এর ক্ষেত্রে original Service Book সংশ্লিষ্ট বিদ্যালয়ে এবং duplicate Service Book concerned শিক্ষক/শিক্ষিকার কাছে থাকে।

Nomination Form সাথে Subscriber নিজের Identity documents এর copy ও রাখতে পারেন।যেমন-
(ii) Voter Card (EPIC)


সকল Employee এর তার কর্মজীবনের একটি অবশ্যিক ও গুরুত্বপূর্ণ ফর্ম হলো এই Nomination Form. অনেক সময় SI/s কিংবা Head of the Institution (H.O.I) এই বিষয়টাকে গুরুত্ব দেন না। যার ফলে অনেক শিক্ষক/শিক্ষিকা সমস্যায় পরেন, হয়রানির স্বীকার হন। এই Nomination Form fill-up করিয়ে নেওয়ার responsibility সংশ্লিষ্ট SI/s Officials বা H.O.I এর উপর বর্তায়।

Type of Nomination Forms

Nominee যখন খুশি পরিবর্তন কিংবা নতুন করা যায় সেক্ষেত্রে সংশ্লিষ্ট Higher Authority কে Application দিয়ে আগের Nomination Cancel করে নতুন করে এই Form গুলো পূরণ করলেই হবে। Nomination Form 3 ধরনের হয়ে থাকে, নিচে বিস্তারিত দেওয়া হলো।

Form of Nomination-II

General Provident Fund (G.P.F) এর Nomination এর জন্য এই Form fill-up করতে হয় ও General Provident Fund (G.P.F) deduction হওয়ার অব্যবহিত পরেই এই Form fill-up করা যায়।Form fill-up এর গুরুত্বপূর্ণ point গুলি দেখানো হলো, যাতে কোন প্রকার ভুল-ভ্ৰান্তি না ঘটে-
  • Account Number:
এখানে P.F Account number লিখতে হবে। প্রাথমিক শিক্ষক /শিক্ষিকাগণ SI/S Office থেকে কিংবা DPSC থেকে পাওয়া Annual G.P.F Statement এ এটি পেয়ে যাবেন। High School এর ক্ষেত্রে বিদ্যালয় থেকেই P.F Account number পাওয়া যাবে।
  • Name of Subscriber:
তারপর যিনি Subscriber তার নাম লিখতে হবে।
  • Name and  Full address of the Nominee(s):
এখানে Nominee বা Nominee গণের নাম ও ঠিকানা দিয়ে পূর্ণ করতে হবে।
  • Relationship with Subscriber:
এখানে Subscriber এর সঙ্গে Nominee এর সম্পর্ক কি তা স্পষ্ট ভাবে লিখতে হবে।
  • Age of the Nominee (s):
Nominee বা Nominee গণের বয়স।
  • Share payable to each Nominee (s): 
এখানে কোন Nominee কে কত % (শতাংশ) Share করলেন তা লিখতে হবে।
  • Contingencies (আকস্মিক) on the happening of which the Nomination will become invalid
এখানে যাকে Nominee করেছেন তার Nomination কোন কোন কারণে Cancel (বাতিল) হবে অর্থাৎ Death (মৃত্যু), Divorce (বিচ্ছেদ), Insanity (উন্মাদ)
  • Name, address and relationship of the person or persons, if any, to whom the right of nominee shall pass in the event of his/her predeceasing the subscriber
 
যদি Subscriber এর পূর্বে Nominee এর মৃত্যু হয়, সেক্ষেত্রে Nominee এর অধিকারী যিনি হবেন সেই ব্যক্তির নাম, ঠিকানা ও Subscriber এর সাথে কি সম্পর্ক তা লিখতে হবে।
  • If the Nominee is not a member of the Family as provided in para-4 (f) indicate the reason
Nominee যদি Family member না হয় সেক্ষেত্রে 1995 এর P.F Rules এর Para 4(f) অনুযায়ী Reason দেওয়া হয়। এখানে para-4(f) তে Family বলতে কাদের বোঝানো হয়েছে তা নীচে দেওয়া হলো-

(f) Family means-

(i) in the case of male subscriber, the wife or wives and children of a subscriber and the widow or widows and children of deceased son of the subscriber, provided that, if subscriber proves that his wife been judicially separated from him or has ceased under the customary law of the community to which she belongs to be no entitled to maintenance, she shall thenceforth be deemed (গণ্য) to be no longer a number of the subscriber's family in matters to which these rules relate, unless the subscriber subsequently indicates by express notification in writing to the Head of the Institution that she shall continue to be so regarded;

(ii) in case of a female subscriber, the husband and children of a subscriber, and the widow or widows and children of deceased son of the subscriber;

Provided that if a subscriber by notification in writing to the Head of the Institutions expresses her desire to exclude her husband from her family, the husband shall thenceforth be deemed to be no longer a member (সদস্য) of the subscriber's family in matters to which these rules relate, unless the subscriber subsequently cancels formally in writing her notification excluding him.

Note-1. 'Children' means legitimate Children.
Note-2. An adopted child shall be considered to be child when the Head of the Institution, or if any doubt arises in the mind of the Head of the Institution, the Government is satisfied that under the personal low of the subscriber adoption is legally recognized as conferring the status of a natural child, but in this case only.

  • Date of Option of Nominee:
যেদিন Nominee করার option দেওয়া হলো, সেই তারিখটা লিখতে হবে-
Dated this 5th day of September 2020 at 3:00 pm তারপর Institution এর নাম যেমন-Ramipur Primary School
  • Signature of Witness:
এখানে দুইজন Witness এর নাম ও Signature করতে হবে।Primary School বা High School এর দুইজন সহকর্মীর Name ও Signature করাতে হবে।
Primary School এ দুইজন Staff না থাকলে ঐ Circle এর অন্তর্ভুক্ত যেকোনো বিদ্যালয়ের শিক্ষক/শিক্ষিকাকে দিয়ে এই Witness করানো যাবে।
  • Signature of the Employee:
Signature of the Employee/Subscriber এর জায়গায় Subscriber Signature করবেন ও নীচে Block Letter এ নাম ও Designation লিখবেন।
  • নীচের বাকি কাজগুলো সংশ্লিষ্ট Office এর 
Nomination by Sri/Smt. এর স্থানে যিনি Nomination দিলেন তার নাম fill-up করবেন।এরপর Signature of the Head of the Institution (H.O.I) এর জায়গায় Primary School ক্ষেত্রে SI/s Signature এবং Seal দিয়ে Designation ও Date উল্লেখ করবেন।High School হলে একইভাবে Headmaster (H.M)/Assistant Headmaster (A.H.M) /Teacher in charge (T.I.C) সবকিছু করবেন।শেষে Dated of Receipt of Nomination এর তারিখ হবে যেদিন Nominee Form এ Signature করবেন, সেই দিন।

Enclosure-D

Nomination for payment of Death cum Retirement Gratuity

চাকুরীতে যোগদান করার অব্যবহিত পরেই এই Form পূরণ করা যায়।যেহেতু Death Gratuity-এর ক্ষেত্রে 1-দিন চাকুরী করে মারা গেলেও Death Gratuity পাওয়া যায়, তাই হয়রানি থেকে বাঁচতে প্রথমেই এই Form fill-up করে নেওয়াই যুক্তিযুক্ত। Retiring Gratuity এর ক্ষেত্রে এই Form-টি 10 বছর চাকুরী পূর্ণ হওয়ার পর fill-up করার নিয়ম, তথাপি এটিও আগে থেকে fill-up করেরাখা ভালো। Form fill-up এর অংশ গুলো নিম্নে দেখানো হলো -

1. Name and address of the Nominees:
এখানে Nominee এর নাম ও সম্পূর্ণ ঠিকানা দিয়ে পূরণ করতে হবে। 

2. Relationship with Employee:
Employee এর সঙ্গে Nominee এর সম্পর্ক উল্লেখ করুন। 

3. Age of Nominee:
 Nominee এর বয়স বা জন্ম তারিখ (Date of Birth) ও লিখতে পারেন।

4. Amount of share of Gratuity payable to each: 
যারা যারা Nominee থাকবেন, তাদের প্রত্যেকে Gratuity এর কত শতাংশ (%) Share করা হলো তার পরিমান লিখুন।যদি ঐ Employee একজনকেই সবটা দিতে চান, তাহলে 100% লিখতে হবে।

5. Contingencies on the happening of which the Nomination will become Invalid:
যাকে Nominee করেছেন তার Nomination কোন কোন কারণে বাতিল (Cancel) হতে পারে-Nominee র মৃত্যু হলে (Death), বিচ্ছেদ হলে (Divorce) বা উন্মাদ হয়ে গেলে (Insanity)।

6. Name, Address and relationship of the person or persons, if any, to whom the right conferred on the Nominee shall pass in the event of the nominee pre-deceasing the Employee or the nominee dying after the death of the Employee but before receiving Payment
 
যদি Employee এর মৃত্যুর আগে Nominee মারা যান অথবা Employee এর মৃত্যুর পর Nominee তার টাকা পাওয়ার আগেই যদি মারা যান, তাহলে সেক্ষেত্রে যিনি Nominee এর অধিকারী হবেন তার Name, Address ও Employee এর সাথে তার সম্পর্ক লিখতে হবে।

7. Amount of Share of Gratuity payable to each: 
এখানে Column No.4 এর মতোই হবে।

Nomination Cancel

Nomination Cancel এর ক্ষেত্রে "The Nomination supersedes the Nomination made by me earlier on ...... ...... ..... which stands cancelled" স্থানটি পূর্ণ করতে হবে।Nomination Cancel হওয়ার পর পুনরায় আবার নতুন করে Form fill-up করে জমে করতে হবে। বাকি অংশের Form fill-up আগের মতোই। Form of Nomination-II এর মতো। যেঅংশ গুলো প্রয়োজন নেই সেগুলো Across Line টেনে কেটে দিতে হবে।

Payment of arrears of Pension (Nomination) Rules, 1986

Form-A
Pension এর ক্ষেত্রে এই Form পূরণ করতে হয়।এক্ষেত্রেও এই Form-টি 10 বছর চাকুরী পূর্ণ হওয়ার পর fill-up করার নিয়ম, কারণ 10 বছর চাকুরী পূর্ণ না হলে Pension পাওয়া যায় না।এই Form fill-up এর বিভিন্ন অংশ নিচে দেখানো হলো -
  • Place:
 School এর Name, Address এবং Circle বা DI/S এর অন্তর্ভুক্ত।
  • Pensioner এর নাম Capital Letter এ লিখতে হবে।
  • Name and Address of the Nominee:
Nominee এর Name ও Address লিখতে হবে।
  • Relationship with Pensioner:
 Pensioner এর সাথে Nominee এর কি সম্পর্ক, লিখতে হবে।
  • Date of Birth (DOB):
Nominee এর জন্ম তারিখ বসিয়ে পূর্ণ করতে হবে।
  • If Nominee is minor Name and Address of Pensioner who may receive the said Pension during the Nominee's minority  
Nominee যদি Minor হয়, সেক্ষেত্রে ঐ সময় যিনি Pension পাবেন তার Name and Address লিখতে হবে।
  • Name and Address of other Nominee in case the Nominee under Column (1) predeceases the Pensioner  
আরও একজন Nominee এর Name and Address দিতে হবে। কারণ যদি Pensioner এর আগে Nominee মারা যান তার জন্য। 
  • Relationship with Pensioner:  
Pensioner এর সাথে ঐ ব্যাক্তির সম্পর্ক লিখতে হবে।
  • Date of birth if the other Nominee is minor:
Nominee যদি minor হয় তাহলে তার Date of Birth (DOB)/জন্মতারিখ লিখতে হবে। 
  • Name and address of person who may receive the pension (পেনশন) during the other Nominee's minority 
Nominee এর Minor হলে, Minor থাকাকালীন যিনি Pension এর দাবিদার হবেন তার Name ও Address লিখতে হবে।
  • Contingency (আকস্মিক) on happening of which Nomination shall become invalid:
কি কি কারণে Nomination Cancel হবে।Nominee র মৃত্যু হলে (Death), বিচ্ছেদ হলে (Divorce) বা উন্মাদ হয়ে গেলে (Insanity)।
  • Place:
Place এর স্থানে High School এর ক্ষেত্রে School এর নাম। Primary School এর ক্ষেত্রে School এর নাম এবং Circle name লিখতে হবে। 
  • Date:
Nomination Form যে দিন জমা দেওয়া হলো, সেই তারিখ উল্লেখ করতে হবে।
